We do everything possible to make your admission and stay in hospital a comfortable and positive experience. Our friendly and highly-skilled administrative and medical teams will guide you every step of the way to help meet both your medical and emotional needs while you are in our care.

What to do before you’re admitted
Clerical Pre-Admission
Before you can be admitted to St Luke’s Care Private Hospital, you must complete St Luke’s Care Pre-Admission Forms in consultation with your doctor, confirming your personal details, consent for treatment and the details of your procedure.
These forms need to be completed and returned to St Luke’s Care Private Hospital at least 7 days prior to your hospital admission. This allows enough time for the Pre-Admissions Nurse to review your details and address any identified concerns.
Nursing Pre-Admission
The Pre-Admissions Nurse may contact you by telephone prior to your admission. If you are contacted, your medical history will be reviewed with you, and admission and discharge details discussed. Please take this opportunity to discuss any concerns regarding your admission.
Completing the Pre-Admission Forms
To fill out the Pre-Admission forms, you will need:
- Personal and Next of Kin details
- Medicare Card
- Payment details (e.g. DVA, Private health insurance, self-funding)
- Benefit details (e.g. PBS Safety Net Card, Pension Card)
- Item Numbers; if these were provided by doctors’ rooms
- Details of your medical history (e.g. current medications, prostheses or implants)
The Pre-Admission Forms also contain information about your stay with us such as what to discuss with your doctor, health insurance and payment information, what to bring, meals, hospital facilities and more.
Your doctor will notify the hospital of the date of your procedure/operation and inform you of your day and time of admission. Your doctor will also explain your procedure or operation.
The Day Before Your Admission
The day before your admission, if we have not already contacted you, please ring (02) 9356 0200 after 2:00 pm to confirm your admission and be allocated an arrival time.
Generally, you should not eat or drink for at least six (6) hours before your operation/procedure, unless otherwise notified by your doctor, anaesthetist or hospital staff.
Please ensure that you bring all current medications with you.
The Day of Your Admission
Please check in with the Front Reception in the main foyer of the hospital at the time indicated by your doctor, and you will be directed to your admission area.
The Admitting nurse will review your medical history and discharge information with you. This is also a good time to discuss any concerns or questions you may have.
The hospital will inform you of the scheduled time for your surgery and subsequent ‘nil by mouth’ time. This means nothing to eat or drink, including water, prior to your operation.
Please bring with you into hospital anything applicable to your admission, including:
– Doctor’s admission letter
– Health fund number/details (if applicable)
– Medicare card
– Any entitlement cards, e.g. Safety Net, Veterans’ Affairs, Pension Card. If not presented, full costs will be charged
– Current medications in their original containers and prescriptions, including repeat forms
– Relevant x-rays, scans, pathology/blood test results or films
– Payment for estimated gap between fund benefits and hospital fees, or total estimated costs of hospitalisation if you do not have private health insurance
– Any paperwork not already forwarded to the hospital
– Sleepwear, dressing gown and slippers
– Toiletries and personal articles, e.g. sanitary pads (if applicable)
– Comfortable day clothes
– Any aids, such as walking stick and hearing aids
– For a child; favourite toy, formula, bottle and any special dietary needs (if applicable)
DO NOT (unless advised by your doctor):
– Eat or drink anything after midnight the night before for morning surgery
– Eat or drink anything after 7 am for afternoon surgery; a light breakfast prior to 7 am is acceptable e.g. tea and toast
– Smoke cigarettes or chew gum
– Wear jewelry (a wedding ring and watch are permitted)
– Bring valuables
– Wear make-up or nail polish (including shellac / acrylics etc.)
Day procedure patients (additional information):
– Please shower on the day of admission, before coming to the hospital
– Wear garments that are clean, comfortable and easy to remove
– Check with your nurse before informing relatives/friends of the time that you should be picked up
